The following information is required to register your child at Reyburn Intermediate:

Parent or Legal Guardian must be present

Proof of Residency (Must be in Parent’s/Guardian’s name)-One of the following:

Recent PG&E bill
-
Recent Water, Sewage and Garbage bill
-
Current Rental Agreement
-
Recent Property Tax payment receipt

Recent Pay Check Stub
-
Escrow Papers that indicate closing within 45 days

Voter Registration Card
-
A certified copy of the Voter Registration Application form

Current Immunization information:
-
PPD/Mantoux – one documented at time of enrollment in California school
-
Tdap – on or after 7th birthday

Certified Birth Certificate
-
Unofficial Transcript (Drop slip if registering during the school year)
